Assisted Living Accommodation Care Model
Our assisted living accommodation currently has a vacancy, and we're excited to introduce you to our innovative care model. We believe that everyone deserves the chance to grow their independence, even if they weren't successful in obtaining approval for SIL or ILO. Here's how our care model works:
Personalised Suitability Assessment
Before you join our community, we'll conduct a thorough suitability assessment. This process ensures that you meet the functional requirements to respond to emergencies, manage behaviours, and maintain house rules. It's all about ensuring your safety and the safety of your co-tenants. During the assessment, we collaborate closely with your care team, we will require clinical assessment reports, such as an OT Functional assessment report, NDIS Plan, and other therapy and diagnosis reports.
Tailored 1:1 Care
Once you're part of our community, we provide personalised 1:1 care tailored to your unique needs and preferences. Our aim is to empower you with full control over setting your care plan. Unlike traditional Supported Independent Living, where you may need to conform to standardised routines, we empower you to set the rules for how you're cared for. All our staff undergo the screening and specialised training that is directly linked to you and your care.
Conflict Resolution and Boundaries
We understand that conflicts may arise in shared living situations. With our care model, we place a special focus on matching personalities that are compatible with one another. We will work with you to ensure you have an effective conflict resolution strategy in place, and we help you establish clear boundaries to ensure a harmonious living environment. We have put in place clear and concise house rules that aim to achieve this outcome.
Support Package
Our support package is centred around you. We offer 1:1 support suited to your specific needs, and 1:3 overnight support. Your well-being is our top priority. At Carepro Disability Service, we're committed to helping NDIS participants achieve their goals, live independently, and have the quality of life they deserve.
